The Practice provides paid vacation benefits to eligible employees that consistently work twenty-four (24) hours or more per week.

Paid vacation benefits are provided to full-time employees who have completed one (1) year of continuous employment at the Practice.

Accrual of Vacation

Accrual of vacation begins as of completion of one (1) year for full-time employees.

Vacation accrues during an employee’s active service with the Practice. Vacation days accrue as service is performed. Employees will not be given retroactive credit for any period of time in which they do not accrue vacation days because they were at the maximum. Active service commences with an employee’s first day of work and continues thereafter unless broken by an absence without pay, a leave of absence, or termination of employment. Moreover, vacation days do not accrue during unpaid leaves of absence or other periods of inactive service.

Eligible employees accrue vacation based on years of service, per year worked according to the following schedule based on the anniversary year:

1—3 years’ service                              40 hours per year

3—5 years’ service                              80 hours per year

5+ years’ service                                  120 hours per year

Carryover or Payout

Vacation may not be carried over to the following year. Earned and unused vacation is paid out at the end of each year while you are employed.

Separation from Employment

Upon termination of employment, employees will not be paid for vacation days that have accrued but remain unused through the last day of work.

Requesting Vacation and Documentation

Employees may request to take accrued vacation after one (1) year of employment, as work schedules permit. Vacation schedules must be coordinated and cleared with the doctor and should be requested at least three (3) months in advance. The Practice schedules determine permissible vacation periods, which employees may need to defer or otherwise adjust accordingly.

Vacation requests are on a first come first serve basis. If possible, they are to coincide when the doctor takes her vacation.

All vacation requests must be documented and approved through HRFH Leave of Absence requests.

Mandatory Use of Vacation

The Company ceases operation during the week of Christmas. Employees are required to use accrued vacation during this Company shutdown.

Doctor Out of Office: Notice

There will be times when the doctor is out of the office for Practice business, continuing

education, or personal reasons. You will be given as much notice as possible of the dates and times when the doctor will be away from the office. If your presence is not required, you may either take time off without pay, or utilize accrued unused vacation. Please notify management of your decision within one week of the time when the doctor will be out.

Rate of Pay and Overtime

For nonexempt employees, vacation time is paid based on the employee’s straight time pay rate in effect at the time the vacation is taken. Vacation is not considered time worked for the purpose of calculating overtime for the week in which the vacation was taken.
